Healthcare Plan Basics

Our three medical plans all include prescription drug coverage and free preventive care.

Each plan works with a pre-tax savings account — either a Flexible Spending Account or Health Savings Account. It's important to understand some basics.

Deductible

Your deductible is a flat dollar amount of covered healthcare expenses that you pay out of pocket before expenses are shared through coinsurance. Note that premiums do not count toward the deductible.

True Family Deductible (CDHP)

The CDHP features a True Family deductible, which means that if you cover any dependent(s), you must meet the full Family deductible before coinsurance begins for any covered individual.

Coinsurance

When your covered healthcare expenses exceed the deductible, you and the plan begin sharing the cost. In all three options, your in-network coinsurance is 20%.

Out-of-Pocket Maximum

The out-of-pocket maximum is the total amount you could pay within a plan year for eligible medical care and prescriptions. Once you reach this maximum, the plan pays 100% of covered services for the rest of the year.

In-Network vs. Out-Of-Network

Using in-network providers is a smart strategy for keeping your healthcare costs as low as possible. Out-of-network providers are not held to the same pricing guidelines. Consider this example for an ACL knee injury:

Qualifying Life Event (QLE)

Annual Enrollment is the only time you're permitted to make changes to your coverage unless you experience a qualifying life event. Examples include: birth/adoption of a child, marriage or divorce, or a dependent becoming eligible or ineligible for coverage. QLE benefit changes must be made within 30 days of the event.

Similar to the UnitedHealthcare options, Kaiser offers three plan tiers:

Kaiser Essential HMO

Lower premium option with standard deductible and copays. Works with a Healthcare FSA.

CDHP

Kaiser CDHP with HSA

Consumer-directed plan with HSA eligibility and Asurion matching contributions. Works with a Dental & Vision FSA.

Enhanced

Kaiser Enhanced HMO

Higher premium option with lower deductible and out-of-pocket costs. Works with a Healthcare FSA.

Note on HMO Coverage

As an HMO, Kaiser requires you to use Kaiser network providers and facilities. You must select a Kaiser primary care physician who coordinates your care.

Prescriptions

Prescription coverage is included with your medical plan. You will receive a single ID card.

Formulary

All pharmacy benefit partners use a regularly updated list of medications called a formulary to inform participants which prescriptions are considered preventive (and often free), preferred, or requiring special authorization to fill. You always may ask your doctor or pharmacist whether a lower-cost alternative prescription is available.

Generic versus Brand Medications

Generic medications are proven to hold the same potency and results as brand equivalents. Whenever they're available, generic medications are substituted for brand medicines unless your doctor indicates "Dispense as Written" on the prescription. If either you or your doctor requests a brand-name medicine when a generic is available, you are responsible for paying the cost difference.

Step Therapy

Step therapy is a strategy a doctor may use when a patient starts taking certain medications. Monitoring the patient's results with the medication before changing the dose or the prescription may be necessary, especially if a medication could be costly or cause undesirable side effects.

Prescriptions in the CDHP

If you're enrolled in the CDHP, prescriptions are subject to the medical plan deductible — you pay the full cost of non-preventive medications until you reach the annual deductible. Preventive generic medications are $0 under the CDHP. You can use your Health Savings Account to pay for prescriptions.

Maintenance Medications

Maintenance medications are prescriptions intended to be taken regularly for more than 90 days. You may receive up to two 30-day refills at a network retail pharmacy, but you must then request a 90-day prescription filled via mail order through Optum. Otherwise, you are responsible for the full cost.

Participants with Diagnosed Medical Conditions

Optum is integrated with Livongo for diabetes and Hello Heart for hypertension care. For patients with serious health concerns, an individualized care plan is available, including a concierge service to schedule and coordinate medical appointments and help monitor prescription adherence.

Dental

Dental care is an important part of an overall preventive care strategy. Lack of good dental hygiene has been linked to heart attack and stroke.

Priority #1: Always get care in-network!

Steps to Keep Costs Low

1. Establish a relationship with an in-network PCP

The first visit with a new doctor costs up to 50% more than a return visit. Avoid multiple first-time-visit charges by seeing one PCP who can get to know you personally and help manage your health.

TIP: If you use your free annual physical for your first visit with a new doctor, you can avoid the higher first-visit costs altogether.

2. Take advantage of free preventive care

The Affordable Care Act requires that medical plans cover 100% of the cost of annual physicals and other age- and gender-appropriate preventive screenings, such as well-child exams, mammograms, colonoscopies and prostate screenings — whether or not you've met your deductible.

3. Use telemedicine instead of seeing your PCP

Online consultations with board-certified doctors cost about the same as a PCP office visit without traffic or waiting rooms. Telemedicine is available 24/7 for non-emergent health concerns, including mental health, through your phone, computer or tablet.

4. Avoid getting care at a hospital (except for true emergencies)

Get X-rays, MRIs, CT scans and other imaging tests at independent facilities. Non-hospital-affiliated imaging centers can provide identical services for a fraction of the cost.

TIP: A non-emergency ER visit can cost you hundreds of dollars more than a PCP office visit. Consider telemedicine or urgent care whenever possible.

5. Use generic medications where possible

Generic medications meet the same high FDA standards and contain the same active ingredients, strength, safety standards and quality. Ask your doctor to prescribe a generic medication whenever possible.

6. Locate your closest in-network urgent care center

Urgent care and walk-in clinics provide a convenient, lower-cost alternative for non-emergency care after hours and on weekends.

7. Use specialists only when referred by your PCP

Specialists can charge more than double the cost of a typical office visit. Also, make sure your PCP refers you to an in-network specialist.

8. Use national laboratory groups for blood tests

Similar to choosing your pharmacy, you can ask for a written lab order and take it to the lab of your choice. National laboratory groups have significantly lower prices.

9. Max out your healthcare savings

The tax advantages available through FSAs and HSAs are hard to beat. Even if you're a healthy person, unexpected healthcare needs can pop up anytime. Prepare now!

Life insurance above $50,000 is taxed by the federal government. The value of the premiums as determined by the IRS is reported as imputed income.

Consumer Directed Health Plan (CDHP)

A CDHP is a specially designed plan that puts you, as the consumer of healthcare, in the driver's seat. The key differences from a traditional PPO are:

  1. Copayments vs. Coinsurance: In a PPO you pay a copayment to see a provider. In the CDHP, you pay all of your cost of care out of pocket until you reach the deductible.
  2. HSA Access: The CDHP allows you to use a Health Savings Account. If you save per paycheck, you'll be ready if high healthcare costs hit.

Can I Have an HSA and FSA?

Yes! If you choose the CDHP with HSA, you may also open a Dental and Vision FSA — because you'll use your HSA for medical and prescription expenses.

Flexible Spending Accounts

Save pre-tax money through payroll deductions to cover healthcare and/or dependent daycare expenses. Note: You forfeit any money not used before the annual deadline.

Healthcare Flexible Spending Account

You may save up to $3,300 in pre-tax contributions for qualifying healthcare expenses. The total amount you elect is available on January 1, but remember — use it or lose it. There is no rollover feature with an FSA.

Dependent Daycare Flexible Spending Account

Save money to cover child and elder care expenses through convenient pre-tax payroll contributions. You can use these funds to pay for preschool, summer day camp, before/after school programs, and eligible child or elder care.

Note for Highly Compensated Employees (HCEs)

The IRS prohibits Dependent Daycare FSA plans from favoring HCEs. If you are deemed an HCE, your election may be limited. You will be notified if there is any impact to you.


Commuter Flexible Spending Account
