Choose the healthcare coverage that aligns with your needs and life situation.
You have three plan options, all through UnitedHealthcare's broad network, giving you flexibility and wide access to providers.
What it is: Copay plan that leverages a Preferred Provider network.
Who it fits: Perfect for individuals and families who prefer a lower premium with the trade-off of a higher deductible.
What it is: Consumer-directed health plan that works with a Health Savings Account (HSA).
Who it fits: Perfect for both high and low users of the medical plan, especially those who want to save tax-advantaged funds.
What it is: Copay plan that leverages a Preferred Provider network.
Who it fits: Great for those who don't mind paying extra premiums for lower deductibles, out-of-pocket maximums and PCP copayments.
For those who prefer a more integrated care model (HMO), Kaiser is available in six regions through its narrow network. Kaiser plan provisions vary by region. Check your eligibility for Kaiser coverage on the ABC.
Our three medical plans all include prescription drug coverage and free preventive care.
Each plan works with a pre-tax savings account — either a Flexible Spending Account or Health Savings Account. It's important to understand some basics.
Your deductible is a flat dollar amount of covered healthcare expenses that you pay out of pocket before expenses are shared through coinsurance. Note that premiums do not count toward the deductible.
| Individual | Family | |
|---|---|---|
| Essential PPO | $4,000 | $8,000 |
| CDHP with HSA | $2,500 | $5,000 |
| Enhanced PPO | $1,000 | $2,000 |
The CDHP features a True Family deductible, which means that if you cover any dependent(s), you must meet the full Family deductible before coinsurance begins for any covered individual.
When your covered healthcare expenses exceed the deductible, you and the plan begin sharing the cost. In all three options, your in-network coinsurance is 20%.
The out-of-pocket maximum is the total amount you could pay within a plan year for eligible medical care and prescriptions. Once you reach this maximum, the plan pays 100% of covered services for the rest of the year.
| Individual | Family | |
|---|---|---|
| Essential PPO | $8,000 | $16,000 |
| CDHP with HSA | $5,000 | $10,000 |
| Enhanced PPO | $3,500 | $7,000 |
Copays are a fixed amount for a covered service collected when you see your provider (unless you have the CDHP).
| PCP (Primary Care) | Specialist | Urgent Care | |
|---|---|---|---|
| Essential PPO | $30 | $50 | $60 |
| CDHP with HSA | Deductible, then coinsurance | ||
| Enhanced PPO | $25 | $50 | $50 |
Using in-network providers is a smart strategy for keeping your healthcare costs as low as possible. Out-of-network providers are not held to the same pricing guidelines. Consider this example for an ACL knee injury:
| In-Network | Out-of-Network | |
|---|---|---|
| Provider Billed | $10,000 | $10,000 |
| Network Discount | $6,500 | Not applicable |
| Amount Allowed | $3,500 | $4,200 |
| Health Plan Paid | $2,800 | $3,360 |
| Your Responsibility | $700 | $6,640 (incl. balance bill) |
Annual Enrollment is the only time you're permitted to make changes to your coverage unless you experience a qualifying life event. Examples include: birth/adoption of a child, marriage or divorce, or a dependent becoming eligible or ineligible for coverage. QLE benefit changes must be made within 30 days of the event.
Asurion offers voluntary benefits to round out your coverage. Learn more about Accident, Critical Illness and Hospital Indemnity voluntary coverage options.
Get more out of your coverage with these important next steps.
| What to do | Why you should do it | Where to find it |
|---|---|---|
| Verify newly added dependent(s) | Documentation is required. Otherwise, your new dependent won't be added to your medical coverage, and there won't be another opportunity until Annual Enrollment (unless you have a qualifying life event). | After submitting enrollment, you'll see a task in your To-Do List on the ABC home page. Reach out to the ABC (844.968.6278) with questions. |
| Download your insurance carrier's mobile app | If you haven't received your ID card, you can download your carrier's app for easy access to your ID card, claims information and telemedicine provider. | Download from Apple or Android app store. You'll need to create a username and password. |
| Adjust your Health Savings Account balance | Your contributions are too low or too high? You can adjust anytime during the year. | Visit the ABC. |
| Report a life event | You've added a child, gained other coverage, or changed your marital status. You've got 30 days to act! | Visit the ABC. |

Kaiser HMO plans available in six regions for those who prefer integrated care.
Kaiser is available in six regions through its narrow HMO network. Kaiser plan provisions vary by region. Check your eligibility for Kaiser coverage on the ABC.
Kaiser HMO coverage is available in select areas. Please log in to Asurion Benefits Central to verify your eligibility based on your location.
Similar to the UnitedHealthcare options, Kaiser offers three plan tiers:
Lower premium option with standard deductible and copays. Works with a Healthcare FSA.
Consumer-directed plan with HSA eligibility and Asurion matching contributions. Works with a Dental & Vision FSA.
Higher premium option with lower deductible and out-of-pocket costs. Works with a Healthcare FSA.
As an HMO, Kaiser requires you to use Kaiser network providers and facilities. You must select a Kaiser primary care physician who coordinates your care.
Prescription coverage is included with your medical plan. You will receive a single ID card.
All pharmacy benefit partners use a regularly updated list of medications called a formulary to inform participants which prescriptions are considered preventive (and often free), preferred, or requiring special authorization to fill. You always may ask your doctor or pharmacist whether a lower-cost alternative prescription is available.
Generic medications are proven to hold the same potency and results as brand equivalents. Whenever they're available, generic medications are substituted for brand medicines unless your doctor indicates "Dispense as Written" on the prescription. If either you or your doctor requests a brand-name medicine when a generic is available, you are responsible for paying the cost difference.
Step therapy is a strategy a doctor may use when a patient starts taking certain medications. Monitoring the patient's results with the medication before changing the dose or the prescription may be necessary, especially if a medication could be costly or cause undesirable side effects.
If you're enrolled in the CDHP, prescriptions are subject to the medical plan deductible — you pay the full cost of non-preventive medications until you reach the annual deductible. Preventive generic medications are $0 under the CDHP. You can use your Health Savings Account to pay for prescriptions.
Maintenance medications are prescriptions intended to be taken regularly for more than 90 days. You may receive up to two 30-day refills at a network retail pharmacy, but you must then request a 90-day prescription filled via mail order through Optum. Otherwise, you are responsible for the full cost.
Optum is integrated with Livongo for diabetes and Hello Heart for hypertension care. For patients with serious health concerns, an individualized care plan is available, including a concierge service to schedule and coordinate medical appointments and help monitor prescription adherence.
Dental care is an important part of an overall preventive care strategy. Lack of good dental hygiene has been linked to heart attack and stroke.

What you'll pay for coverage is just one consideration. Think about whether you have kids (who often need braces), and consider your family's dental history — if a parent or sibling has needed dental services, there's a good chance you may too.
Your premiums are deducted from your paycheck before taxes. You can pay your deductible and coinsurance with a Healthcare FSA, HSA (CDHP only), Dental and Vision FSA (CDHP only), or personal debit/credit card.
Get a free hearing assessment and up to 68% off hearing aid options.

You're investing money in your healthcare along with Asurion. Use your benefits efficiently and wisely.
The first visit with a new doctor costs up to 50% more than a return visit. Avoid multiple first-time-visit charges by seeing one PCP who can get to know you personally and help manage your health.
The Affordable Care Act requires that medical plans cover 100% of the cost of annual physicals and other age- and gender-appropriate preventive screenings, such as well-child exams, mammograms, colonoscopies and prostate screenings — whether or not you've met your deductible.
Online consultations with board-certified doctors cost about the same as a PCP office visit without traffic or waiting rooms. Telemedicine is available 24/7 for non-emergent health concerns, including mental health, through your phone, computer or tablet.
Get X-rays, MRIs, CT scans and other imaging tests at independent facilities. Non-hospital-affiliated imaging centers can provide identical services for a fraction of the cost.
Generic medications meet the same high FDA standards and contain the same active ingredients, strength, safety standards and quality. Ask your doctor to prescribe a generic medication whenever possible.
Urgent care and walk-in clinics provide a convenient, lower-cost alternative for non-emergency care after hours and on weekends.
Specialists can charge more than double the cost of a typical office visit. Also, make sure your PCP refers you to an in-network specialist.
Similar to choosing your pharmacy, you can ask for a written lab order and take it to the lab of your choice. National laboratory groups have significantly lower prices.
The tax advantages available through FSAs and HSAs are hard to beat. Even if you're a healthy person, unexpected healthcare needs can pop up anytime. Prepare now!
Save your money by choosing the right level of care.

A CDHP is a specially designed plan that puts you, as the consumer of healthcare, in the driver's seat. The key differences from a traditional PPO are:
Yes! If you choose the CDHP with HSA, you may also open a Dental and Vision FSA — because you'll use your HSA for medical and prescription expenses.
| Health Savings Account (HSA) | Dental & Vision FSA |
|---|---|
| Available only with the CDHP with HSA. ✅ Unused balance rolls over year to year. ✅ Use now, save for later, or invest. ✅ Covers medical, Rx, dental, and vision expenses. |

Save pre-tax money through payroll deductions to cover healthcare and/or dependent daycare expenses. Note: You forfeit any money not used before the annual deadline.
You may save up to $3,300 in pre-tax contributions for qualifying healthcare expenses. The total amount you elect is available on January 1, but remember — use it or lose it. There is no rollover feature with an FSA.
Check out FSAstore.com to browse an online store full of products that qualify as eligible expenses.
If you're enrolled in the CDHP with HSA, you can instead contribute pre-tax money to a Dental and Vision FSA (for qualified dental and vision expenses only).
Save money to cover child and elder care expenses through convenient pre-tax payroll contributions. You can use these funds to pay for preschool, summer day camp, before/after school programs, and eligible child or elder care.
The IRS prohibits Dependent Daycare FSA plans from favoring HCEs. If you are deemed an HCE, your election may be limited. You will be notified if there is any impact to you.
